Descripción (Español)
El clonazepam (distribuido por Roche bajo las marcas Klonopin en los Estados Unidos y Rivotril en Europa, Sudamérica, Canadá, India, y Australia) es un fármaco de tipo benzodiacepínico, con acción depresora del sistema nervioso central, y propiedades ansiolíticas y anticonvulsivantes.
Farmacocinética
Se absorbe en el tracto gastrointestinal. La eliminación del fármaco es lenta ya que los metabolitos activos pueden permanecer en la sangre varios días e incluso semanas, con efectos persistentes. El clonazepam es de vida media intermedia, fluctuando entre las 30 y las 40 hs. Su unión a las proteínas es alta; se metaboliza en el hígado y se excreta por vía renal.
Indicaciones
El clonazepam se indica para el tratamiento de: crisis mioclónicas; ausencias de tipo epiléptico (refractarias a succinimidas o ácido valproico); crisis convulsivas tonicoclónicas (generalmente asociado con otro anticonvulsivo); trastornos de pánico, o trastornos del sueño como el sonambulismo.
Contraindicaciones
Clonazepam no debe utilizarse en pacientes con antecedentes de hipersensibilidad a las benzodiazepinas, con evidencia clínica o bioquímica de enfermedad hepática significativa ni con insuficiencia respiratoria severa. Puede emplearse en pacientes con glaucoma de ángulo abierto sometidos a una terapia adecuada, pero está contraindicado en el glaucoma agudo de ángulo estrecho.
Reacciones adversas
Los más frecuentes son somnolencia, mareos, dificultades en la coordinación psicomotora (ataxia) y en la función motriz, deterioro en las funciones cognitivas, y depresión.
También pueden presentarse nerviosismo, alteraciones del comportamiento, cansancio y debilidad no habituales, pérdida del equilibrio, y amnesia anterógrada (especialmente con dosis altas).
Los pacientes geriátricos y debilitados, niños y pacientes con trastornos hepáticos, son más sensibles a los efectos de las benzodiazepinas sobre el SNC.
El clonazepam, y todos los agentes benzodiazepínicos en general, tienen contraindicación parcial en casos de ADHD, ya que tienden a producir un efecto paradójico en este grupo de pacientes, exacerbando los síntomas de hiperactividad e impulsividad, además del potencial efecto negativo sobre la atención, la memoria de trabajo y las funciones ejecutivas en general. No obstante, tales decisiones farmacoterapéuticas deben evaluarse en cada caso en particular.

Contraindications
Klonopin should not be used in patients with a history of sensitivity to benzodiazepines, nor in patients with clinical or biochemical evidence of significant liver disease. It may be used in patients with open angle glaucoma who are receiving appropriate therapy, but is contraindicated in acute narrow angle glaucoma.
How to use
